How We Support You

We provide high quality continuity of healthcare, and help adults living with a disability caused by acquired brain injury, neurological conditions or psychosocial conditions, to meet their goals and live fulfilled lives.

We work with the individual to undertstand their goals, identify their needs, and explore their interests and new passions. We simply the goals, breakdown actions to easily achievable steps, prepare easy plans, to achieve individual goals.

We identify and address any barriers to achieving the goals which could include measures that up-skill or re-skill, restore relationships, enhance community engagement/access, facilitate courses, facilitate new experiences, provide emotional support, etc.

Core Support

General Disability Support includes all services that we provide to help you live a stress-free and enjoyable life as independently as you want. The support services we provide include but not limited to:

  • Personal Care and Grooming (bathing, dressing, etc.)
  • Daily Personal Activities (feeding, toileting, recreation, companionship, etc.)
  • Domestic Assistance (meal preparation, cleaning, laundry, gardening, lawns etc.)
  • Community Access (shopping, sports, GP visits, family visits, out-door recreational activities etc.)

 

The above support services can be provided daily for as long as required (including 24/7), and can also be provided In-Home, Independent Living Option (ILO), or in Supported Independent Living (SIL).

Nursing Services

HyCare’s community nursing services assists individuals with health care needs that require the skills of a registered nurse. The support provided is tailored to meet your specific health needs and you will be equipped with the skills to manage your health independently as much or as little as you want.

The services we provide include but not limited to:

  • Chronic Disease Management
  • Medication Administration
  • Wound Care
  • Continuity of Healthcare
  • Care Planning & Nutrition
  • Health Monitoring, Guidance, and Counselling

Therapy

Our Specialist Support services includes all the therapeutic and intervention services aimed at improving your physical, cognitive, and mental well-being, and to increase your ability to participate in daily activities. You can use your NDIS funding to access a range of therapeutic supports that are tailored to your specific needs.

The services we offer in-house include but not limited to:

  • Occupational therapy
  • Mental health support
  • Innovative community participation
  • General therapy

 

There are a range of the specialist services offered through our partners – we make the referrals on your behalf to take away the stress so you can focus on your well-being.

Wellbeing & Capacity Building

We understand the significance of social, mental, emotional, and spiritual determinants of health and wellbeing. Our wellbeing and capacity building program faciliates social connections, enjoyable occupations, and relaxation activities that enhance wellbeing. We work with participants to identify their interests, explore new passions, identify and break any barriers to achieving their goals. The program involves providing the environment, tools, knowledge, and skills that empower participants with disabilities and their families to take control of their own lives.

The services we provide include but are not limited to facilitating:

  • Recreational activities, holidays, and get aways
  • Community participation, social interaction, and beneficial relationships
  • Skills development, suitable occupations, and new passions
  • Employment assistance, group & centre-based activities
  • Support Coordination (levels 1 & 2)
